PalCare Network has a membership of over 40 hospice palliative care service organizations including York Region’s residential hospice www.hillhousehospice.com, the Central Community Care Access Centre (CCAC), 32 long-term care facilities, 4 hospitals and the Ministry of Health and Long-Term Care.
PalCare Network is proudly funded by the Ontario Ministry of Health Long Term Care Division and the Central LHIN.
